Linagliptin is an inhibitor of DPP-4, an enzyme that breaks down incretin hormones, namely glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). Consequently, linagliptin elevates the concentrations of active incretin hormones, promoting insulin release in a glucose-dependent manner and reducing circulating glucagon levels. GLP-1 and GIP play vital roles in the physiological regulation of glucose homeostasis. Incretin hormones are continuously secreted at basal levels throughout the day, with levels surging immediately after meal consumption. GLP-1 and GIP enhance insulin biosynthesis and secretion from pancreatic beta cells, particularly in the presence of normal and elevated blood glucose levels. Additionally, GLP-1 diminishes glucagon secretion from pancreatic alpha cells, leading to a decrease in hepatic glucose output.
